Project Objectives [from section 2 of PCN] The overall objective of World Bank support is to contribute to the national goal of halting and reversing the AIDS epidemic by attaining the following project development objectives in accordance the national program?s strategic objectives: 1)achieving behavior change for prevention of HIV infection by reducing stigma and scaling up prevention efforts amongst the most at risk populations (MARP) and the general population; 2)increased care, support and treatment of PLHIV. C. Project Description [from section 3 of PCN] The proposed project would have 3 components. Component 1. Strengthening national management capacity to fight HIV infection. Under this component we would support (i) the well-functioning of soon to be established National HIV/AIDS and STI Control Board with technical assistance, equipment and supplies. (ii) The project would also support the management capacity at regional, district and village (VDC) level. Page 2 (iii) Strategic information management is a key weakness at present. The Bank would support data gathering and analysis capacity. Part of this would be a support for a more comprehensive surveillance system, including behavioral and sero-surveillance. Component 2. Supporting multi-sectoral interventions to fight HIV infection. (i) IEC and Advocacy: the project would support raising awareness and knowledge around HIV for the general population through mass media and more focused means. Focus on the general population is important for various reasons: (a) a large proportion (46%) of PLHIV are migrant worker; (b) although most people are aware of HIV, stigma and misconceptions form an enormous barrier to an effective response; (c) all youth needs to be informed about the hazards of life in a world with HIV. (ii) Support for targeted interventions for the MARPs, complementing the efforts of other agencies. These interventions would relate to prevention, care, treatment and support, including harm reduction for injecting drug users. (iii)Support capacity building efforts amongst NGOs to increase their management skills and effectiveness. (iv) The project would work together with the Poverty Alleviation Fund to make sure that HIV related poverty matters are well taken care of at the community level. Component 3. Supporting the health sector. Care of PLHIV is largely provided in the regular health system. At the same time, health care may itself be a risk factor for HIV infection. Therefore the project would support: (i) logistic support for distribution systems for medical supplies, and (ii) support for blood safety. Hereto, the project will work with the Nepal Red Cross and provide support for capital investments, infrastructure rehabilitation and equipment, for safe blood transfusion services. The precise content of the components will only be defined after Nepal will have received a decision from GFATM regarding its request for grant funding from GFATM round 7. D. No negative impacts on them are expected. We must ensure, however, that the indigenous populations have full access to the services and will benefit in an equal manner from the project. A thourough social assessment will be carried out in order to address the very specific social issues related to the groups with high risk behavior. SAFEGUARD PREPARATION PLAN A. Target date for the Quality Enhancement Review (QER), at which time the PAD-stage ISDS would be prepared: 03/27/2008 B. For simple projects that will not require a QER, the target date for preparing the PAD-stage ISDS: N/A C. Time frame for launching and completing the safeguard-related studies that may be needed. The specific studies and their timing 1 should be specified in the PAD-stage ISDS. The environmnetal assessment and infectious waste management plan will be carried out in the context of the broader ongoing Health Sector Support Project. (Nov07- Jan 08) The social assessment which will include the safeguard issues for indigenous people will be substantial given the specific characteristics of the populations at high risk for HIV infection.(Nov 07-Feb 08) 1 Reminder: The Bank's Disclosure Policy requires that safeguard-related documents be disclosed before appraisal (i) at the InfoShop and (ii) in-country, at publicly accessible locations and in a form and language that are accessible to potentially affected persons.
